To obtain a copy of the discharge summary it is necessary to contact with a passport the hospital department where the treatment was carried out.
To obtain written responses to inquiries from law enforcement agencies, industrial enterprises, and other legal entities for obtaining medical documents please contact the hospital office (administrative building, 1st floor, office 2, tel. 245-28-21 - head of the office from 09:00 to 16:00 (lunch 13:00-13:30)).
Employees of law enforcement agencies, industrial enterprises, and other legal entities to obtain medical history, other medical documents,need to contact the hospital office and present:
- identity document;
- written inquiry on institutional letterhead.
Inpatient medical records are issued in person only to law enforcement officers upon presentation of the above documents along with a written inquiry!
To obtain X-ray images it is necessary to contact the X-ray room located in the admission department of the City Clinical Center for Traumatology and Orthopedics clinic (Building 4, 1st Floor).
X-ray images are recorded on the recipient's electronic media!
