The use of PRP in orthopaedic surgery
Promotion of healthy lifestyle in recent times leads to a sharp increase in the number of athletes both professional and Amateur level. In this regard, there is a jump in morbidity and injuries in the musculoskeletal system of competitors.
In the city center of Transfusiology of the 6th city clinical hospital for effective treatment of various injuries and exacerbations of chronic conditions of the musculoskeletal system with great success the PRP-therapy method is used.
PRP-therapy (plated rich plasma-English.: platelet-rich plasma) is a method of treating various types of diseases by injecting autologous platelet-rich plasma isolated from the patient's whole blood.
The principle of operation is based on the delivery to the pathological zone of natural factors from alpha granules platelets involved in the body's recovery processes such as:
- epidermal growth factors (EGF);
- vascular endothelial growth factors (VEGF);
- platelet derived growth factors (PDGF):
- transforming growth factors beta (TGF-b) and the growth factor of fibroblasts (FGF)
The method of PRP therapy is most effective when:
- Exacerbation of degenerative-dystrophic diseases of the musculoskeletal system (arthrosis and arthritis of various joints, spinal osteochondrosis, etc.).);
- shoulder-scapular periarthritis (damage to the rotator cuff, tendon of the long head of the bicep, impigment syndrome;

- tendinitis, ligamentitis (inflammation of the Achilles tendon, inflammation of the patella's own ligament).

The use of special tubes by the type of "hourglass", as well as adapted centrifuges allow to obtain preparations of platelet-rich plasma in the shortest possible time (on average, the procedure takes no more than 20 minutes). The concentration of platelets in such samples according to laboratory studies can reach 2 000 000 activated platelets in 1 ml of the drug (for comparison, in whole blood this number is an average of 250 000 not activated in 1 ml).
Blood treatment by the method used leads to platelet concentration 7-9 times, which leads to their activation – the release of growth factors from alpha granules. The resulting platelet layer is taken from the test tube and is used as a therapeutic drug by injection into the affected segment of musculoskeletal apprata. The course of injections consists of 3-7 procedures with a frequency of every 7-10 days, depending on the type of disease.
